Minimum Requirements
- Bachelor of Science in Physical Therapy from an American Physical Therapy Association approved program, or equivalent degree.
- Current state Physical Therapist license in Massachusetts required to start the assignment.
- Proof of current CPR certification required.
- Current driver’s license, reliable transportation, and auto insurance required.
- 1-2 years of experience as a Physical Therapist in an acute care or rehabilitation setting required.
- Previous home care experience preferred.
- Ability to perform heavy lifting, bending, pulling, pushing, prolonged standing and walking required.
- Ability to work a flexible schedule and travel locally as needed.
- Must read, write, and speak fluent English.

- Candidate must be able to perform evaluations, develop and update Plans of Care, document progress notes, and collaborate with interdisciplinary team members.
- Must be able to instruct patients and families on transfers and safe use of assistive devices and to order or adapt rehabilitative equipment for home environments.
- May be required to travel locally and float to other service locations as needed.
