Minimum Requirements

• Bachelor's, Master's or Doctoral Degree in Physical Therapy from accredited schools approved by the APTA
• Active State License is Required to Start the Assignment
• BLS Certification May Be Required from AHA or ARC
• Current Physical Therapy licensure in the state of Oklahoma
• Current CPR certification
• Must have a current driver's license and vehicle insurance, and access to a dependable vehicle

• Facility Type: Home Health
• Schedule flexibility is needed in this role
• Participation in a weekend rotation and potential availability on observed holidays is required
