Minimum Requirements

  • Bachelor of Science in Physical Therapy from an APTA-approved program, or equivalent professional degree (required).
  • Current, active South Carolina Physical Therapist license or ability to obtain state licensure prior to the assignment start (required).
  • Proof of current CPR certification (required).
  • Current valid driver’s license, reliable transportation, and vehicle insurance (required).
  • 1–2 years experience as a Physical Therapist in an acute care or rehabilitation setting (required).
  • Home health experience (preferred).
  • Demonstrated knowledge of durable medical equipment and ability to order and adapt equipment for home environments (required).
  • Ability to perform clinical assessments, prepare and revise Plans of Care, document progress notes and phone orders, and collaborate with interdisciplinary teams (required).
  • Ability to perform physical tasks including lifting, bending, pushing, pulling, prolonged standing and walking (required).
  • Commitment to continuing education and ability to meet mandatory CE requirements (required).


Assignment Details

  • Facility Type: Home Health.
  • Assignment Type: Travel (temporary contractor).
  • Start Date: Assignment begins April 2026 and extends through July 2026.
  • Clinical Scope: Treat patients across the lifespan for neuromuscular, neurological and orthopedic diagnoses; provide patient and family education, transfer training, and DME recommendations.
  • Documentation: Prepare and maintain POC, progress notes, and other clinical records per agency standards.
  • Local Travel: Must be able to travel locally to patient homes and work a flexible schedule as needed.
