Minimum Requirements
- Doctorate in Physical Therapy from an accredited program. Graduate from an accredited school of Physical Therapy is required.
- Minimum 2 years current Physical Therapist experience required. New graduates will not be considered.
- Active state PT license. Facility will pend license upon offer. Must apply within 24 hours of offer acceptance. Compact license is not acceptable for this need unless specifically listed as required by the facility.
- Active American Heart Association BLS certification required.
- Ability to treat common outpatient diagnoses, including post-operative care, joint replacement, back pain, tendonitis, deconditioning, balance and vertigo, neurological disorders, pulmonary rehabilitation, and pelvic floor dysfunction.
- Previous rehabilitation outpatient experience required. Previous experience working with PTAs preferred.
- CPSI EMR experience highly preferred.
- Previous travel experience strongly preferred.
- Must be able and willing to float and provide inpatient PT coverage as needed.
- Traveler must have own vehicle. This is a firm requirement due to limited public transportation in the community.
- Profile and documentation requirements prior to submission: updated CAQH after start, NPI after start, work history, skills checklist current to within one year (document), one supervisory signed reference within past two years (document), traveler permanent address included in profile. Requested time off (RTO) must be noted at upload. No more than 5 days RTO will be accepted; requests after upload will not be approved.

- Shift Pattern: Variable shifts within a 7:00 a.m. to 6:00 p.m. window, 40 hours per week
- Weekend Requirement: None routinely. Holiday coverage if required.
- Typical patient volume: Approximately 8 evaluations/treatments per day Monday-Thursday and 6 per day on Friday.
