Job Title: Speech Language Pathologist (SLP)

Reports To: Director of Rehabilitation
Status: Full time, Non-Exempt, Non-Bargaining

Job Summary: The Speech Language Pathologist (SLP) will carry out an individualized program of Speech Language Pathology including assessment, treatment, planning, implementation, education, and communication to maximize a patient's progress toward achieving functional goals. The SLP will also maintain a positive working relationship with other health professionals and ancillary staff. This position requires skills in patient/client evaluation and treatment, as well as professional consultations to achieve maximum patient/client independence in accordance with the individual’s functional level and potential. The individual shall perform all duties in a manner that promotes team concept and is reflective of Kahuku Medical Center’s mission.

Supervisory Responsibilities:
Potential supervision of support staff including aides and/or assistants
Case management of patient caseload

Duties and Responsibilities:

Provides direct patient care, evaluates functional needs and outcomes, and consults with other specialists. Reviews patients’ functional needs and adjusts care plans to ensure optimal patient outcomes.
Designs and implements a plan of care for patients based on a thorough assessment of potential development and incorporates participation by significant others whenever it seems appropriate.
Conducts therapy practice within defined standards of care.
Directs the assistance of support staff to achieve patients’ functional goals.
Communicates effectively via written, verbal, and digital communication with other professionals and support staff to achieve positive patient outcomes.
Practices hospital and departmental policies and procedures, objectives, quality improvement program, safety, environmental, and infection control standards.
Efficiently documents evaluations, treatment goals, and plans. Regularly updates documentation and maintains prescriptions and signatures. Documentation is to be completed in the Hospital’s electronic medical record system
Documentation shall be completed within the established parameters of the department
Enhances professional growth and development of self and others through participation in educational programs, in-service meetings, and workshops. Keeps up to date on professional literature.
Provides direction and leadership to staff and acts as a resource to promote Speech Language Pathology in the community.
Punctuality and professionalism are key attributes to the position. The expectation is to arrive for shift on time and ready to receive the first patient on time. The expectation is also to make all preparations to leave on time at the end of the day. Overtime will not be granted unless previously approved.
Strict adherence to all department and Hospital policies and procedures
Perform other duties as assigned

Required Knowledge/Skills/Abilities:

Knowledge and skills to care for the physical and developmental needs of the population age groups that will be receiving services.
Ability to remain calm in emergency or crisis situations.
Flexibility is a key aspect of the position. The therapist must be willing to be flexible during patient care and willing to work with the hospital team. The therapist may be required to flex between hospital and outpatient care.
Documentation
Efficient and effective documentation is a requirement of the position. A therapist shall complete documentation during the therapy day. The quality of documentation shall also reflect an appropriate subjective report, education provided, assessment of the treatment, plan for the next session, and appropriate charge delineation
It is expected that documentation of patient appointments, daily notes, and evaluations be initiated point of contact
Evaluations and daily notes shall be completed on the same day of service
All notes will be completed in the hospital’s EMR
Must be able to communicate effectively with the team. Each team member has a responsibility to communicate information regarding a patient’s Plan of Care at team meetings, through daily documentation, and through other electronic communication. This also requires the therapist to perform some administrative caseload duties.
Knowledge and use of Microsoft Office 365 is preferred.

Education and Experience:
REQUIRED
Graduate of an accredited Speech Language Pathology program.
Current active license from the State Board of Speech Language Pathology.
Current active Basic Life Support (BLS) certification

PREFERRED
At least one year of experience in post-acute or outpatient setting is preferred. However, new graduates are very welcome to apply.

Physical Requirements
Prolonged periods of sitting at a desk and working on a computer;
Must be able to lift 50 pounds at times;
Must be able to access and navigate each department at the organization's facilities.

Pay: $43.51 - $51.26 per hour
