The Registered Nurse (RN) Outreach Liaison is responsible for all external relationships including, but not limited to Hospitals, Facilities, and Providers. Actively maintains, communicates, and facilitates problem solving between external relationships and LSRVNA. Achieves assigned targets for volume and type of admissions necessary to support the agency’s budget and strategic plan.

This position requires travel within our service area. The right candidate for this role will live within a reasonable commute to New London, NH.

Responsibilities for the Registered Nurse (RN) Outreach Liaison:

Keeps informed regarding all LSRVNA programs and services and is prepared to speak on behalf of LSRVNA
Actively supports LSRVNA efforts to improve and maintain communications with providers and the community
Facilitates providing education to external relationships regarding LSRVNA programs and services
Achieves targets for volume and type of admissions established by supervisor
Informing LSRVNA staff of changes within the community, referral sources and competitors that will impact LSRVNA services and demand
Monitors community/client/hospital expectations of LSRVNA and levels of satisfaction; communicates with other organizations and with appropriate personnel in this regard
Supports the referral process and the transition of patients to LSRVNA services from referral sources by:
Initiating collection and dissemination of necessary data to appropriate staff for establishing, revising, and updating client records
Assists in identification of the need for new or additional printed materials which promote LSRVNA and assists in the development of such materials
Attends in-services/conferences required for the position/program as appropriate and initiates requests to attend training to improve job performance
Identifies problems and opportunities and offers constructive suggestions/solutions
Develops and submits reports as requested
Engage in respectful communication (both verbal and written) with team members, patients, family members, and the community
Adhere to agency wide policies
Other duties as assigned

Qualifications for the Registered Nurse (RN) Outreach Liaison:

Marketing/Sales experience preferred
Experience in the Healthcare setting preferred
Homecare experience preferred
Current NH RN license required
Excellent written and oral communication skills required
Proof of Flu vaccination
