RN Homecare Quality and Performance Improvement Manager

Masonicare Home Health & Hospice - Connecticut


Day Shift / 40hrs/wk 


Manages, facilitates, and implements the quality management and quality improvement programs and infection surveillance at Masonicare Home Health & Hospice to achieve quality outcome goals. Understands and follows the quality and regulatory standards for the position, department and agency.  

Essential Duties and Responsibilities:

 

  1. Champion’s quality improvement processes throughout Masonicare Home Health & Hospice, ensuring a standardized and unified approach to achieve documentation compliance and outcome goals [Value Based Purchasing -Rehospitalization; satisfaction; quality OASIS outcomes; star ratings].

 

  1. Manages clinical oversight of coding and 485 review to facilitate quality outcomes and documentation compliance with CoPs, state regulations, coding standards and payor requirements.

 

  1. Manages auditing of OASIS review outsource to facilitate timely, accurate and consistent performance; escalate performance concerns impacting quality outcomes. 

 

  1. Uses Strategic Healthcare Programs software to support review of documentation.

 

  1. Discusses any noted documentation discrepancies with Directors, clinical managers and clinical staff as appropriate. 

 

  1. Participates in quality and committee activities as necessary.

 

  1. Identifies and facilitates education regarding the key quality initiatives/targets/goals (based on federal, state and best practice guidelines) for the organization.

Facilitates appropriate documentation and OASIS education as indicated.

 

  1. Tracks infection trends for employees and patients based on DPH and CDC guidelines and participates in Masonicare Infection Committees

 

 

  1. Tracks, analyzes, and collects data for the purpose of demonstrating the efficacy of our improvement processes.

 

  1. Develops Performance Improvement projects based on key performance indicators and facilitates implementation in the agency.

 

  1. Assists with development of yearly and long-term quality management plans, including required and specialty audits based on service delivery trends and/or general agency/industry trends. 

 

  1. Manages and assists with clinical record review and general chart audits per the PI Plan, federal and state regulations and guidelines.

 

  1. Participates in the completion of and presents quality reports as requested.

 

  1. Participates in branch staff meetings as needed. 

 

  1. Works closely with Risk Management in regards to the tracking of sentinel and/or adverse events and translating this to the QI process.

 

  1. Responsible for processing additional documentation requests (ADRs) and appealing denials.

 

  1. Participates in requisite committee/board meetings, including the Professional Advisory Committee meetings

 

  1. Participates in CMS and DPH audits/visits/investigations as required/requested.

 

  1. Assists with the professional development of assigned staff, including participation in orientation, collaboration with education on necessary in-services, ongoing skill development and other mentoring activities.

 

  1. In conjunction with the Director and Human Resources, participates in staff recruitment and selection of potential hires.

 

  1. Understands the mission of Masonicare by recognizing the population we serve as unique persons, requiring individual alternatives to maintain full lives in their later years

 

 

Minimum Qualifications:

 

Education:                  Minimum of Associates Degree in Nursing from an NLN accredited college or university. Baccalaureate Degree preferred.

 

Experience:                 Five (5) years’ experience in acute care/home care/public health administration/SNF. Two (2) years’ experience in quality management preferred.

 

Certificates, Licenses, Registrations:             Connecticut Registered Nurse license required. Maintains current Connecticut Drivers’ License and auto insurance.
