Job Summary
Health Navigators will be assigned to specific employer clients and will manage the employee population specifically in their healthcare needs; this may require the Health Navigator travel to client locations within the greater Houston area to provide health education and counsel to employees one-on-one and in group settings. The Health Navigator will assist employees with support such as locating in-network providers, identifying appropriate levels of care, managing chronic conditions, preventative care plans, healthy lifestyle habits, education resource management, and maximizing their benefit offerings. The Health Navigator will engage in proactive outreach to client employee population and provide broad engagement events and activities to promote program participation. MINIMUM QUALIFICATIONS
Education: Graduate of an accredited School of Professional Nursing; Minimum Associates Degree of Nursing required; Bachelors of Nursing preferred
Licenses/Certifications:
Current State of Texas license or temporary/compact license to practice professional nursing required
Certified in Basic Life Support required
Additional certifications may be required based on discipline and/or nursing unit requirements
Experience / Knowledge / Skills:
Minimum three (3) years of clinical nursing experience required
Strong clinical background in nursing, additional experience in social services is a plus
Motivational interviewing training and experience is strongly desired
Experience with electronic charting required, Epic experience is a plus
Knowledge of insurance terminology
Basic knowledge of MS Office including MS Outlook
Excellent verbal and written communication skills
Ability to perform multiple tasks simultaneously, work under pressure, and meet critical deadlines
Exceptional documentation skills
Ability to work independently, manage time, and prioritize projects
PRINCIPAL ACCOUNTABILITIES
Adheres to all regulatory and Texas Board of Nursing standards.
Engages with employees in their workplace to support them in navigating their healthcare goals and concerns including managing chronic conditions, understanding new diagnoses, preventive health screenings, weight management, and improving nutrition and/or setting health goals.
Meets with individual employees regularly and provides extended visit times to allow in-depth discussions and to identify and reduce barriers to care through the use of motivational interviewing techniques and an individualized approach.
Assists employees with setting and attaining health-related goals and acts as an accountability partner to the employee as they endeavor to achieve and maintain health-related goals.
Provides biometric measurements including weight, BMI, blood pressure, A1c, lipid, and glucose.
Assists employees with locating in-network healthcare providers and may assist with coordinating care among providers.
Promotes the Health Navigator program and builds case load via proactive engagement of appropriate employees through outreach and the development and delivery of broad engagement activities and presentations.
Partners closely with the employer’s HR/benefits team(s) to understand the employer-sponsored benefits and resources in order to appropriately guide employees in navigating in-network care according to the referral methodology defined by the employer.
Completes and maintains appropriate documentation and reporting.
Participates in meetings and presentations to communicate and report process in relation to program metrics and outcomes.
Participates in a systemic, interdisciplinary, and ongoing evaluation of programs, process improvement and desired client-centered outcomes.
Ensures safe care to patients, staff and visitors; adheres to all Memorial Hermann policies, procedures, and standards within budgetary specifications including time management, supply management, productivity and quality of service.
Promotes individual professional growth and development by meeting requirements for mandatory/continuing education and skills competency; supports department-based goals which contribute to the success of the organization; serves as a resource to less experienced staff.
