MJHS is a large not-for-profit health system in the Greater New York area. Our range of health services include home care, hospice and palliative care for adults and children, rehabilitation and nursing care at Menorah and Isabella Centers, and the research based MJHS Institute for Innovation and Palliative Care. We also offer Elderplan/HomeFirst: health plans for Medicare and dual-eligible individuals. We offer a broad range of services in the community or facility-based to help patients and their families ease the burden of managing a life-limiting illness or end of life care.
- Mentors and provides leadership direction to the Social Work staff
- Social Worker Documentation Compliance
- Participates in Referral and Community Education and Presentations
- Oversees the preparation and planning of weekly team meetings
- Master's Degree in Social Work from a Graduate School of Social Work accredited by the Counsel of Social Work Education
- Minimum five years post Master's degree
- Minimum 2 years end-of-life care experience
- Minimum 2 years of supervisory/program development experience
- New York State LCSW licensure within one year of employment, R # preferred
- SIFI Certification within one year of employment
- Valid driver license preferred
