About the Job:
Staff RN - Hospice, within the scope of Ohio Nurse Practice Act, will provide care in an atmosphere sensitive to each person's physical, emotional, social and spiritual needs. All hospice care will take place in patient homes.
Hospice Staff RN serving as a hospice care navigator for patients hospitalized for GIP Care.
This position will require travel to the hospitals in Central Ohio area.
What you'll do:
- Nursing Process: Assesses and evaluates patient care for an assigned group of patients, utilizing evidence based practice. Collaborates with patient and family to develop and implement a plan of care that includes physical, spiritual and educational components.
- Communication: Accurately communicates pertinent information to appropriate team members in a timely manner. Keeps patient/family apprised/updated
- Documentation: Documents accurately and completely pertinent assessments, interventions and outcomes for patients in accordance with documentation standards, policies, procedures and/or guidelines.
- Delegation: Assumes responsibility and accountability for care provided and documented by other licensed and unlicensed care givers as appropriate.
- Technology: Utilizes appropriately and demonstrates continuous learning.
- Assists in the orientation of new staff, supports student's experience as appropriate.
- Maintains current knowledge, skills, education and licensure in clinical practice, attends required inservices and staff meetings, and demonstrates professional growth.
- Participates in a performance improvement approach that includes both intradepartmental and interdepartmental activities.
What we are looking for:
- Experience: Minimum of 2 years work experience as a professional nurse
- Ability to operate a motor vehicle required; possesses a valid driver’s license from the State of Ohio and has access to a functional automobile during work hours.
- Requires use of a personal vehicle with valid auto insurance.
- Current clinical experience, preferably in community home health or hospice setting.
- Criminal background check and fingerprinting completed as required by Ohio law.
- Education: Graduate from school of nursing. Baccalaureate degree in nursing preferred
- Licensure: Current license to practice in the state of Ohio
