This Quality Coordinator/RN Coordinator position is full time, offer full benefits and a convient day shift.  The Quality RN /Care Coordinator is a registered nurse who functions in a leadership role in Population Health.  S/he is a liaison between the patient and the multidisciplinary healthcare team to assist in developing and implementing a patient centered plan of care for high risk patients.The Quality Coordinator RN assesses patients’ needs and coordinates resources within and outside the practice to promote optimal health benefits and quality outcomes in a manor most acceptable to the patient. S/he works in direct coordination with the local Quality Team Team and serves as an educational resource for providers.  S/he works with the Quality Department to analyze and communicate population health data to meet the Triple Aim of Population Health.

 

 

Job Duties/Responsibilities:

  • Contacts high risk patients within 2 business days of discharge to assure a safe landing
  • Assesses for community resources at home and arranges for services
  • Creates patient centered care plan to avoid hospitalizations and emergency department visits
  • Assists the clinic in scheduling the TOC visit in 7 to 14 days
  • Follows up with patients for 30 days post discharge as clinically necessary

Qualifications

Requriements:

  • Graduation from an accredited nursing education program.  Licensed by the State to practice as a Registered Nurse.  Training in Motivational Interviewing preferred.

  • Bachelor's Degree
  • Master's Degree, preferred.
  • Minimum of 5 years in case Management, Home Care or Hospice required.

  • Motivational Intervening, Shared Decision Making, Patient Centered Medical Home principles.

  • Certified Case Manager (CCM), Certifies.  Critical thinking, problem solving, ability to work both independently and in teams, and ability to coach and mentor clinical and non-clinical staff.
