Job Summary:

A team member of nursing service, that participates in the delivery of health care to the patients of Pueblo Community Health Center.

Education/Experience/License/Certification:

Registered Nurse with a valid Colorado license.
Current B.L.S. with skills demonstration C.P.R. certification for Healthcare Providers required.
Previous experience with pediatrics/adolescent and adult ambulatory care preferred.
Valid driver’s license and auto insurance as required by law.

Required Travel:

Occasional local travel may be required.

Essential Duties and Responsibilities:

Adhere to the guidelines and procedures of Pueblo Community Health Center, Inc.
Maintain current knowledge of and appropriately integrate use of case management programs within department.
Schedule patient appointments using Next Gen EPM..
Maintain knowledge of medical provider patient appointment template and use for scheduling.
Maintain current knowledge of Triage Protocols and documentation within EMR using Triage Template per Triage MA\Nursing policy and procedure.
Manage all Patient/Nurse Visits per MA\Nursing P & P.
Maintain current knowledge of Next Gen EHR nurse and provider work flows and use for providing care.
Manage Inbox and complete messages same day received if in clinic. Manage Provider’s Inbox per MA\Nursing Policy and Procedure.
Review medication refill requests, and using medication refill guidelines and protocols, refill medications to appropriate pharmacy, or task to provider for review.
Manage providers daily schedules including scheduling and working in acute appointments based on triage protocols.
Assess symptomatic patients and present to provider for direction. Scribe for provider when indicated.
Review normal and abnormal labs and radiology results with patients per provider direction and Nursing policy and procedure.
Order labs and/or radiology per protocol or provider direction.
Complete forms (as appropriate) for providers per Nursing policy and procedure.
Serve as an information resource for MA’s in the pod.
Promote effective communication and teamwork within integrated team including MA’s, Care Coordinators, providers, and all entities involved in patient care team.
Assist a Medical Provider in providing direct patient care. This will include the following but not limited to:
Anthropometrics – Document in vital signs template
Maintain appropriate patient care forms in patient medical record per EMR outage Policy and Procedure.
Assist with treatments, surgical procedures, and emergencies.
Prepare patient for examination or treatment.
Provide information to the patient concerning preparations for laboratory/radiographic examinations.
Serve as an informational source to the patient concerning Specialty Physician referrals, Medical Triage, and Medications prescribed by the Physician.
May fax Radiology requisitions to appropriate hospital, if it does not require Prior Authorization from patient insurance otherwise task to Referral Dept.
Schedule acute appointments for specialty referral if patient is to be seen within 48 hours, Referral dept must be tasked referral for insurance authorization.
May schedule and fax PT\OT\ST\ and home health as directed by the medical provider.
Administration of immunizations/medications as prescribed by the physician and documented.
Perpetuate and anticipate the needs of the medical provider within the flow of patient care.
Responsible to abstract patient (paper) medical records into the Electronic Health Record (EMR) if not complete per MA\Nursing Policy and Procedure on established patients.
Have new patients who have never been seen to fill out Past Medical History Form to be abstracted into their Electronic Health Record prior to seen by Provider.
Document all Patient Encounters within their EMR per policy and procedure.
Manage Inbox and Work flow in a timely manner. All patient calls or tasks received from the Provider must be complete by same day received. any medication
Match unmatched prescriptions with the Providers Inbox
Manage Symptomatic Telephone call during evening clinic from assigned Providers Inbox and use Electronic Triage Assessment and complete per MA\ Nursing Policy and Procedure.
Perform patient exams as EKG, Spirometer, Audiogram, HGB, Serial Glucose, HIV, Urine HCG and Routine U/A.
Responsible to keep patients up to date with immunizations per Chief Medical Officer’s standing orders.
Responsible to schedule children for well child exam if they are behind.
Pack and sterilize surgical instruments per MA\ Nursing policy and procedure.
Adhere to Nursing Policies and Procedures.
Be a team member and participate in ongoing evaluations of systems.
Provide nursing health care services appropriate to the age-specific needs of all patients in accordance with the principles of growth and development for infant, child, adolescent, adult and geriatric life cycles.
Adhere to the Infection Control Policies and Procedures.
Maintain current licensure and CPR certification.
Perform other related duties as assigned by Colorado Nursing Manager.
Follow organization and departmental call off procedure.
The employee may be required to provide services to an individual who is not a patient of the health center when the provider is then asked, called upon, or undertakes at or near the location of an emergency to provide service to a non-health center patient in an emergency situation or temporarily treat or assist in treating the non-health center patient.
The employee may be required to provide supervision of students and medical residents providing care at the health center’s facilities.
Responsible for carrying walkie to respond to stat/ critical labs or provider to provider calls during work hours
Attend weekly mandatory meetings
Daily huddle with patient care team
Access daily reports through CIIS and AZARA for care team
