LTSS SERVICE CARE MANAGER

Pay Range: $23 - $26/hour. Employee benefits include, but are not limited to, health insurance (medical, dental, vision), 401(k) plan, and paid sick leave (depending on work location).

Key Responsibilities:

• Shift Time & Working Days: Standard business hours by time zone 40-hour M-F, No OT required,Monday to Friday.
• LVN needed to assist with follow up calls for members.
• LVN will work assigned tasks
• LVN may perform field visits in Corpus Christi area if needed
• LVN will fill put required forms for each assessment
• Describe the performance expectations/metrics for this individual and their team:
• Complete training and then work turnaround times.
• Tell me about what their first day looks like:
• 8am to 5pm with 1 hour lunch break, meet with manager and get started with training
• What previous job titles or background work will in this role?
• Service coordinator .

Key Requirements and Technology Experience:

• Key Skills:LVN (Licensed Vocational Nurse) is most important Home health experience .
• LVN license.
• Home health experience.
• LVN program completion.
• Microsoft .
