Position Summary
This role is ideal for a PT who enjoys working with a geriatric population in an interdisciplinary, community-focused care model. The Physical Therapist will assess, plan, and implement therapeutic interventions to help participants achieve and maintain their highest possible level of independence and function at home and in the community.
Responsibilities
Schedule / Shift:
- Stable schedule – day shift only
- Provide high-quality physical therapy care as an active member of the interdisciplinary team (IDT).
- Conduct in-person assessments at initial enrollment and as needed, evaluating participants’ functional abilities, home safety, and equipment needs.
- Develop and implement individualized physical therapy treatment plans according to professional standards and participant goals.
- Administer therapeutic programs, monitor progress, and modify interventions as necessary.
- Educate participants, caregivers, and family members on exercise programs, mobility, safety, and home modifications.
Education:
- Advanced degree in Physical Therapy from an accredited program
- Active Kentucy PT license (or eligibility to obtain)
- Minimum of 1 year of experience working with a frail or elderly population preferred; training provided if needed.
- Experience working on an interdisciplinary care team preferred.
- Valid driver’s license and proof of auto insurance required.
