The RN Case Manager will work collaboratively with surgeons and patient/family in developing, implementing and documenting individualized plans of care to ensure continuity, quality and appropriate resource use.


· Plan, develop, assess and evaluate care to patients.

· Recommend alternative levels of care.

· Develop and coordinate discharge plan to meet patients identified needs.

· Review, monitor, evaluate and coordinate patient’s hospital stay to assure that all appropriate and essential services are delivered timely and efficiently.

· Coordinate and provide continuity of care, including utilization management, transfer coordination, discharge planning, and obtaining authorizations/approvals as needed for outside services for patients/families.

· Conduct daily clinical reviews for quality management activities based on guidelines/standards for patients in a variety of settings, including outpatient, emergency room, inpatient facilities.

· Report any incident of unusual occurrences related to quality, risk and/or patient safety which are identified during case review or other activities.


Required:

· 2+ years combined RN and case management experience

· RN License (California)

· BLS

· Working knowledge of regulatory requirements and accreditation standards

· Effective written and verbal communication, interpersonal, critical thinking and problem-solving skills

· Ability in planning, organizing, and negotiating

· Plus: craniofacial exp
