Initiates discharge planning process.Maintains communication with UM coordinator regarding assessments, outcomes, and projected lengths of stay.Serves as a resource to the healthcare team, communicating with the physician, consultants, and all members of the healthcare team.Participates with the physician/multidisciplinary to review the plan and progress of care, to evaluate the effectiveness of interventions and to recommend modification(s) to the plan, as necessary to meet identified patient care goals.Collaborates actively and functions in a multidisciplinary department to provide comprehensive full spectrum care coordination and discharge planning for each patient.Coordinates transfers to appropriate facilities when applicable.Secures authorization for needed additional patient services, as required.Serves as an educational resource to clinical staff around case management.Documents case management assessments and interventions as appropriate, adhering to current standards and policies.Coordinates family and patient meetings in order to develop a comprehensive plan of care during and post hospitalization.Supports the job functions of the other members of the OCM team, as neededConsults with and provides Case Management support to outpatient departments on case-by­ case basis and as time allowsPerforms other duties as required or assigned.Local travelers (Those who reside within 21-75 miles of facility) must be uploaded at a 10% reduction of posted rate.  Facility will not consider any traveler residing within 20 miles of hospital.  Requested time off during the course of this assignment MUST be noted and verified at upload.  RTO REQUESTS AFTER UPLOAD WILL NOT BE APPROVED!!  NO MORE THAN 5 DAYS OF RTO WILL BE ACCEPTED (less is better). Additional Requirements: Must have excellent written and verbal communication skillsMust be able to work as an integral player in a team environmentBasic computer skillsDemonstrated ability to be flexible and able to function under stressful situationsGood organizational and problem-solving skillsClinical Pathways, utilization management, and discharge planning knowledge preferredNon-billable orientation:  0hrsHoliday and OT Multipliers - 1.25% Submission Requirements Minimum 3 years current experience in a CRITICAL ACCESS HOSPITAL setting - RequiredCRITICAL ACCESS HOSPITAL EXPERIENCE STRONGLY PREFERRED - A CRITICAL ACCESS HOSPITAL HAS 25 BEDS OR LESSBSN - HIGHLY PREFERREDMCG experience - PreferredPrior travel experience - PreferredTraveler candidate must have their own vehicle.  This is a firm requirement. Profile Requirements: Work HistorySkills Checklist (pertinent to position), current to 1 year 1 Supervisory Signed Reference (from within the past two years of employment)License/Certifications - Must be uploaded with profile (Nursys, BLS - Facility will not pend license/certs) Certification Requirements BLS (AHA) State License Requirements New Hampshire Compact Allowed Additional License Requirements NH or Compact license must be in-hand at upload.
