The Terraces of Phoenix, a premier 5-Star HumanGood life plan community, is seeking a compassionate and detail-oriented Licensed Practical Nurse (LPN) to serve as our Admissions Nurse and Case Manager within our Skilled Nursing Team. Reporting to the Health Services Administrator, the Admissions Nurse/Case Manager is essential to ensuring a smooth, welcoming, and clinically sound transition for residents entering our community.
** Admissions: 8:30 AM – 5:00 PM, Saturday and Sunday
** Case Manager: 8:30 AM – 5:00 PM, Monday, Tuesday, and Wednesday
** Pay rate: $38.00 - $45.00 (DOE)
Essential job duties and responsibilities include, but may not be limited to:
- Admissions & Intake
- Coordinate and process all new admissions and readmissions to the facility.
- Review hospital referrals, clinical documentation, and insurance authorizations for appropriateness and compliance.
- Complete initial nursing assessments and ensure timely implementation of individualized care plans.
- Verify physician orders, medications, diagnoses, and treatments upon admission.
- Care Coordination & Case Management
- Serve as the primary clinical liaison between hospitals, physicians, families, and internal departments.
- Collaborate with therapy, pharmacy, dietary, social services, and nursing teams to support smooth transitions of care.
- Participate in interdisciplinary team (IDT) meetings and assist with discharge planning as needed.
- Monitor resident progress and support care coordination throughout the stay.
- Documentation & Compliance
- Ensure all admission documentation is complete, accurate, and compliant with federal, state, and facility regulations.
- Maintain accurate and timely records in the electronic medical record (EMR) system.
- Assist with insurance verification, authorizations, and communication with payers.
- Support regulatory surveys, audits, and quality assurance programs.
- Customer Service & Family Support
- Educate residents and families about facility services, care processes, and expectations.
- Respond to admission-related questions and concerns to ensure a positive onboarding experience.
- Quality & Safety
- Promote resident safety and infection control standards during the admission process.
- Report changes in resident condition to providers and nursing leadership promptly.
- Assist with quality improvement initiatives focused on admissions, transitions of care, and re-admission reduction.
What you will need to be successful in these roles:
- Current LPN license in the state of Arizona
- Current CPR Certification
- Experience in skilled nursing, subacute care, admissions, or case management is highly desirable.
- Excellent assessment, documentation, communication, and organizational skills.
- Strong customer service and teamwork acumen.
- Current or prior experience with EMR systems is preferred.
