POSITION SUMMARY
Patient Centered Medical Home staff position responsible for the coordination and care of patients receiving Medication Assisted Therapy (MAT) for opioid addiction. Teaming with a Licensed Behavioral Health Care Professional, the position coordinates health services and supports to a panel of patients in conjunction with prescribing buprenorphine providers within the Central Vermont Health Service Area. Responsibilities include improving the care of the patient population across health, substance abuse and mental health treatment, promoting patient activation and empowerment for shared decision-making in treatment, and encouraging healthy behaviors, self-management of health, mental health, and substance use disorders. In addition, this individual will monitor the health status, treatment progress, and medication adherence of the MAT patient panel. This position will maintain absolute confidentiality of all patient records, medical treatment and diagnosis and abide with all policies and procedures of CVMC.
BASIC KNOWLEDGE
Knowledge of available resources at Central Vermont Medical Center and in the surrounding community to assist with program implementation and care coordination of MAT patients. Excellent interpersonal, communication, and writing skills are essential with the ability to relate and motivate effectively with a diverse group of patients and clinical staff. This position requires advanced knowledge being used in creative and imaginative ways to build a strong sense of commitment to healthy living lifestyles and self-management. Demonstrates computer competency including all Microsoft Office applications.

MINIMUM REQUIREMENTS
RN currently licensed in the state of Vermont. Frequent travel to multiple CVMC locations is required.
EXPERIENCE
3 years of health care experience in field including experience with assessment, motivational interviewing techniques, healthy living, therapeutic lifestyle change and counseling. Electronic medical record experience preferred.
